At this time, we are unable to process most insurance claims, with a few exceptions (see below). As a result, we do not accept co-pays and payment is due at the time of service. However, we do provide each patient with the paperwork including the appropriate codes for reimbursement directly to you from your insurance carrier. You can contact your insurance carrier and they can assist you in submitting your claim for reimbursement.
Sage Health Center does submit claims for TriWest HealthCare VA Choice Program ,First Choice Insurance, Pacific Source, Providence, and some MODA plans. If any of these companies provide health insurance for you, please contact them first about cost, copays and steps to schedule an appointment.
Please keep in mind it is up to each individual to know and understand the details of coverage for their individual insurance plan. The best way to know if a visit/treatment will be covered is to contact your insurance provider directly. We are not responsible for any denial of claims for services provided.
